There is a one time Initiation Fee of $300.00.

• Dues are $27.50 per month ($82.50 per quarter) for full-time 
     and part-time pharmacists.

• Dues are $20.00 per month ($60.00 per quarter) for Per Diem pharmacists.

• Dues are $10.00 per month ($30.00 per quarter) for Pharmacy Interns.

If you are employed at Kaiser and have signed up for Dues Check-Off, your dues will automatically be withheld from your pay check and forwarded to the Guild. However, the Initiation Fee must be paid directly by you and can NOT be withheld thru the Check-Off System.

Dues are otherwise billed on a quarterly basis.

Statement Dates: 

January 1

April 1

July 1

October 1

As an example----the October bill covers the last quarter of the year. 
(Oct., Nov., & Dec.).

Payment is due within 20 days of the date on the statement.
